Ones that I can think of off the top of my head:
Star Wars d20:
Mara Oasaii, Jedi Consular, fell to the dark side and became the Sith Lord the party had to fight.
Rali, former imperial spy who defected to the party. Blew up the engine room while all the other Jedi are fighting Mara and killed everyone.
Pathfinder:
Jessica Armstrong, Paladin who lost her paladin abilities for killing all of a goblin tribe after they surrendered, became a fighter afterwards.
Freeform:
Touka...can't remember her last name. Wouldn't mind returning to her someday.
